Details:
My husband 60yrs old has a neurological injury and onset dementia also dysphasia. He is peg fed but can manage nuticreme pots depending on alertness each day varies. He\'s mobile partially sighted and epileptic but no seizures for 2 hrs. He\'s a funny sense of humour but sadly dementia is onset. Please note phb funding so must be self employed and provide UTR number.
